Caribbean Missions Trip Application

Description: Partnering with one of our two Life Church locations in the Caribbean. The team will do Serve My City outreach by distributing materials and supplies to local churches.

Leader:TBD
Expectation: Bring comfortable clothing/shoes for walking long distances, local projects, and mindful of weather condition.

Travel Time: 2 hours and 30 minutes
Age: 18+ preferred, if individuals 12-17 would like to attend this trip, they must be accompanied with their parent.  

Deposit: $400

Total Cost: $1900
**Total cost may slightly vary depending on airfare and other expenses**

Funding Milestone:
- $500 by 5/1/26
- $500 by 6/1/26
- $500 by 7/1/26

Documents Needed:
- Background Check
- Medical Release Form (notarized)
- Picture of Passport 
- Religious Visa (Department will sign)

Included in trip cost: Airfare, ground transportation, hotel, food, one checked bag, one carry-on and personal item, international emergency medical insurance.

Not included in trip cost: Trip interruption insurance (but you can purchase this separately if you would like, in the event that you expectedly are unable to go on the trip. Additional food or personal expenses. 

PLEASE NOTE:

Payments: All payments and donations are payable to Life Church "Missions Trip Support & Payments". These payments are tax deductible and non-refundable.

Payment Due: All payments are due 2 months prior to departure.

Cancelation: If the minimum headcount for this trip is not met, this trip may be canceled and other options will be provided and/or a refund can be issued or can be transferred to another trip for another time.
